Eligibility for CHC Medicaid waiver

In order to be eligible for CHC waiver services, one must be 21 years of age or older and a resident of Pennsylvania. Additionally, one must be both clinically eligible and financially eligible. Clinically eligible means that the person is determined to be eligible through evaluation of one’s needs from a medical perspective. Financially eligible means having very limited income and resources.

 

Applicants must require a nursing home level of care or be eligible for both Medicaid and Medicare to be deemed eligible for the program. In medical terms of the program that means the person gets a formal assessment and is nursing facility clinically eligible (NFCE).

The process for determining if someone is clinically eligible can be somewhat complex. However, it follows these general guidelines:

    1. The individual has an illness, injury, disability, or medical condition diagnosed by a physician and
    2. As a result of that diagnosed illness, injury, disability or medical condition, the individual requires care and services above the level of room and board, and
    3. A physician certifies that the individual is NFCE(Nursing Facility Clinically  Eligible), and
    4. The care and services are either Skilled Nursing or Rehabilitation Services (For example, a nursing home) or health-related care and services that may not be as complex as Skilled Nursing or Rehabilitation Services, but which are needed and provided on a regular basis through medical and technical personnel.

Aging Waiver

Home and Community-Based Services Waiver for Individuals Aged 60 and Older. Aging Home and Community-Based Waiver Services may be available to Pennsylvanians over the age of 60 to enable them to continue to live in their homes and communities with support and services.

Home and Community-Based Services Waiver for Individuals with Traumatic Brain Injury (TBI). If you have a medically determined diagnosis of traumatic brain injury (TBI), the COMMCARE Waiver can help you remain as independent as possible.

Independence Waiver

If you are an adult with a severe physical disability, the Independence Waiver may be able to help you live or remain in the community and remain as independent as possible.

OBRA Waiver

The OBRA Waiver is a Home and Community Based Waiver program that may help you if you have a developmental physical disability to allow you to live in the community and remain as independent as possible.

Attendant Care/Act 150

If you have a physical disability, the Attendant Care Waiver and state funded Act 150 program may be available to you to continue to live in your home and community with support and services.
